Red Yield / Failure on Numerous Rips

Experiencing numerous errors after a few successful RIPs. All denoted with red yield signs in the Watch Folder View tab. Don't know why. It's very stubborn. I can add these files to iTunes manually through the interface and they work regardless of M4V or MP4 containers, so I know the conversion is working with Handbreak and then breaking as iHomeServer is adding to iTunes.

If anyone has a clue, please share it with me. I searched for the error message and I didn't find on the boards. Is it conceivable that some DVDs could provide issues like?

Application versions below:

WSE2012 - current with all patches
Current stable release tested, current beta release tested. 3.1.76, 3.1.79
Handbreak 0.10.2.7286
AnyDVD 7.6.1.0
iTunes 12.1.2.27

I can provide log file and associated pictures.

In an attempt to use the latest M4V -> MP4 container options in 3.1.79, there was no change in the behavior of the RIP failures.

As of the last attempt with Kill Bill Vol 2, it did not work at all. The drive never opening at the conclusion of the rip and the Conversions list stating perpetually that it's Adding to iTunes. Attempted to intervene by switching to iTunes interactively did nothing (not that I expected that it would).

I ditched trying Kill Bill and moved on to Pleasantville and Mulan. Both worked without any issues. So I'm at a loss.

Odd behavior 1: Attempting to perform a Quick Time Analysis on the the named movies yielded NO result after the ->.
Odd behavior 2: Attempting to perform a Quick Time Analysis on the most recent conversion VTS_0n_n.mp4 yields 2041 Invalid Sample Description.

Disregard the 2013 time stamp on The Fifth Element, as that's a file I attempted to move into the folder for importing into iTunes. But it wasn't incorporated either.

So get this. If I rename the files in the Watch Folder tab to a slightly modified name, they are imported WITHOUT A PROBLEM. What is that all about?!
